We use an individualized, client-centered approach to support families in keeping their loved ones at home.
Medicaid
Planning
Medicaid is a joint federal and state program that helps with medical costs for some people with limited income and resources. It offers benefits not normally covered by Medicare, like Long Term Home Care/Nursing Home Care and Assisted Living. We will assist you in making your loved one eligible for this program while protecting their assets. We will assess to see if your loved one qualifies for Immediate Needs or Institutional Medicaid.
Patient
Advocacy
If you are already receiving services through an MLTC and they are not meeting the needs of your loved one, SAS will initiate a plan appeal for an increase in homecare hours. When needed, SAS is prepared to initiate a fair hearing or explore other options. We do whatever it takes to keep your loved one at home.
Geriatric Care Management
SAS collaborates with your family to create a plan of care that is best suited for your aging loved one. Some examples include Assisted Living Programs, Social Day Programs, NHTD for Dementia, Traumatic Brain Injury ( TBI) Programs, and sourcing talent for home care.
